30-13-207. Application for renewal of assumed business name. One original and one copy of an application for renewal of registration of an assumed business name shall be executed and delivered to the secretary of state. The application shall include but not be limited to the following information:
     (1) the complete assumed business name;
     (2) the name and address, including street name and number, if any, of applicant;
     (3) description of business transacted; and
     (4) the name of the county or counties in which business is being transacted.
