37-41-201. Application for license -- form and contents. Application for a license is made to the department on application forms provided by the department. The application shall contain:
(1) the full name and business address of each applicant;
(2) a list of the counties in which business is to be conducted; and
(3) certification by a local health officer or his designated representative in each county in which the applicant's business is to be conducted that proposed disposal sites to be used by the applicant meet all applicable state and local requirements.
