39-73-105. Application for payment. Application for payment under this chapter shall be made by the person seeking such payment to the department of labor and industry. The application shall be in writing or reduced to writing in the manner and upon the form prescribed by the department. The application form may be filled in and written by a person authorized by the department. If the applicant is unable to sign his or her name on the application, a duly witnessed mark may be used.
