7-3-4210. Form of ballot. At such election the ballots to be used shall be printed upon plain white paper and shall be headed "Special election for the purpose of submitting to the qualified electors of the city of .... the question of reorganization of the city of .... under Chapter 57, Laws of 1911" and shall be substantially in the following form:

[] FOR reorganization of the city of .... under chapter 57, Laws of 1911.

[] AGAINST reorganization of the city of .... under chapter 57, Laws of 1911.
